Rebranding and Care Levels

We are still in the process of rebranding. We can provide a very high level of care in our Assisted Living facility. Being attached to a skilled nursing center allows our residents to age in place.

Community Fee and Utilities

There is a $1,000 one-time community fee. All utilities, including wi-fi and a landline phone, are included in our services.

Included in Base Rate

In our base rate, residents receive all meals, laundry services, and housekeeping.

Rehab and Recovery Option

If someone is discharged by insurance from the care side and is not ready to go home, we offer a Rehab and Recovery option. This is available at a daily rate of $125 plus the cost of care.

Respite Care

We also provide Respite care at the same rate as the Rehab and Recovery option. There is no minimum stay requirement, and this rate is valid for up to 30 days. Both programs include a fully furnished apartment.

Primary Care Availability

Primary care services are available on-site, and a Meet the Team flyer with information about the physicians who visit our community is attached.

Assisted Living Costs

In assisted living, our base rate starts at $3,998 per month. Depending on the level of care needed, additional care costs can range between $370 and $1,970 per month.

Overview of Staff and Training

We employ RNs and LPNs across our community, with both present in the Assisted Living (AL) side depending on the shift. Our team includes aides and nursing aides, all of whom undergo background checks and participate in monthly training sessions. We adhere to nursing home guidelines for all continuing education.

Safety and Well-Being Measures

For the AL side, we conduct assessments to determine the necessary care and safety checks required throughout the day and overnight. We provide pull cords and pendants for residents. Our care side has received a 4 out of 5 stars rating, indicating an Above Average standard. The AL side recently had a state survey with no citations.

Transition Support Services

We offer outings to stores and restaurants to help residents integrate into the community.

Transportation Services

Transportation is available on a first-come, first-serve basis with our van and bus. We assist with alternative transportation arrangements when necessary.

Dining Experience

We are actively working to enhance our dining experience, guided by resident surveys. We offer set dining times, daily specials, and a Bistro menu, with the option to dine in the common area or in residents' apartments.

Resident Grievance Procedure

Our grievance procedure is detailed in our lease agreement. We maintain an open door policy with management for addressing complaints, and residents can also approach the Administrator directly.

Cost of Living

The cost of living in our community starts at $3,998, with additional care costs based on the level of care required.

Family Communication Policy

We conduct care conferences for regular updates and contact families immediately for urgent medical needs via phone.

Short-Term or Respite Stays

We offer short-term or respite stays at a rate of $125 per day, plus care costs.

Pet-Friendly Community

We welcome pets up to 30 pounds.

Currently, we do not have a waitlist for our community.
