Our Facility and Staff

We are a long-term care / skilled nursing and rehabilitation facility, which means we have staffed RNs, LPNs, and STNAs available around the clock. Our team includes a Director of Nursing, Assistant Director of Nursing, and 3 Unit Managers, as well as an Executive Director who oversees the entire facility. We offer licensed physical therapy, occupational therapy, and speech therapy six days a week. Our business office staff includes a Director and assistant, accounts payable, and receptionist staff members. Additionally, we have a licensed MDS worker, licensed social worker, licensed infection control nurse, and medical records staff member. Our dietary department is led by a licensed dietician and a director of dietary, supported by a dedicated staff. We also have a maintenance director, housekeeping, maintenance, and laundry staff, a licensed beautician for hair services, an activities director, transportation director, supply coordinator, licensed case manager, and a robust Admissions team that includes a licensed social worker and an RN Liaison. We are also grateful for the volunteers who assist in our facility.

Training and Safety

Our staff undergoes state-issued and company compliance training throughout the year for all areas of care within our facility. Safety and well-being are paramount for our residents, staff, families, and visitors. We have implemented door codes for secure access and conduct multiple in-services on handling various situations that may arise in nursing facilities.

Discharge Planning and Community Transition

The facility has a discharge planning team that assists residents and families with the transition back into the community. We collaborate with outside agencies to ensure the best care and success for a return to home, striving to make the transition as smooth and flexible as possible.

Transportation and Dining

Transportation services are offered from within our facility by both internal and external resources. We provide multiple dining options, including two dining rooms, patient room dining, and private dining with family/friends. Residents can select their meals from a weekly menu, an alternative menu, or request specific foods. Snacks are available multiple times a day.

Complaints and Concerns

We address all complaints and concerns swiftly through various reporting methods. Once reported, the issue is discussed with the appropriate management teams, a resolution plan is implemented, and the outcome is documented and revisited to ensure effectiveness.

Pricing and Communication

For pricing details, we recommend speaking with our business office. We can provide current room rates, which vary based on room type. Our policy for communication with family members is to inform them of any changes in the resident's condition, adhering to HIPAA regulations and state guidelines. Care conferences are available to discuss concerns, changes, and conditions.

Hospice and Respite Care

Mayfair is contracted with two hospice companies for respite/short-term care services and accepts one-time contracts with other companies. Our policy is to accept respite stays at the Medicare rate per day, with any adjustments requiring written approval.

Pet Policy and Room Availability

We are a pet-friendly facility, with the requirement that shot records are provided for safety. There is currently no waitlist for semi-private rooms, and we can arrange for deluxe rooms. Private rooms have a waitlist.

Vaccinations and Age in Place

Residents and staff members are encouraged to get fully vaccinated for Covid, flu, and hepatitis B. Our facility offers various types of placements and is affiliated with an Assisted Living facility, aiming to assist residents throughout their aging process.
